Eligibility for Application:
Students of the Department of Life Science whose average grade in the 113-2 semester is above 80, from financially disadvantaged families, and with a conduct grade of A or above.

Number of Recipients: 1

Amount: Tentatively NT$6,000 (subject to adjustment based on circumstances).

Application Deadline: Before 5:00 p.m., Tuesday, October 7, 2025. Applicants should submit the completed application form along with all required documents to the Department of Life Science Office, Room 205, Life Science Building II.

Required Documents (please arrange in order):

1. Application form

2. Official transcript of the previous academic year (including class ranking)

3. Household registration transcript (detailed record version) issued within the past three months (Household refers to father, mother, brothers, sisters, and the applicant)

4. Proof of financial disadvantage:

   1. For low-income/middle-low-income households: Submit the government-issued certificate of low or middle-low income for the current year.

   2. For non-low-income/middle-low-income households: Submit (1) household annual income tax documents and (2) household property and income statement.

5. Any other supporting documents for this scholarship (optional if not applicable).
